Booking guide

The booking form helps patients send visit needs and a preferred time in advance. Minh Tam will review the request, contact the patient to confirm a suitable schedule and provide preparation guidance before the visit.

Information to prepare

To complete the form faster, patients should prepare basic contact information and visit needs in advance.

Full name, email and phone number
Date of birth, gender and address if needed
Specialty, package or consultation need
Preferred appointment date and time

Open the booking area

Patients can click Book appointment on the website or go directly to the Contact - Booking section. On specialty or package detail pages, the booking button may scroll to the form and preselect related information when the data is available.

Fill in patient information

The form includes fields such as full name, email, phone number, address, date of birth, gender and ID number if the patient chooses to provide it. Required fields must be completed before the form can be submitted.

Choose the care need

Patients can choose a specialty, package and add a short note if needed. At the moment, the website is configured for one clinic location, so the location field may be hidden until multiple branches are supported.

Choose the preferred date and time

Click the Appointment time field to open the date and time picker. Choose a date, select a suitable time slot and click Confirm selection. The selected time is a request and still needs clinic confirmation.

Agree to the privacy policy and submit

Patients need to confirm that they have read and agreed to the privacy policy. When required information is valid, the Submit button becomes available and the system will show a received status after submission.

Wait for clinic confirmation

Minh Tam staff will review the information and contact the patient through the provided phone number or email. The appointment is considered official only after the clinic confirms it.

Important notes

Do not use the online booking form for emergencies or conditions that need immediate care.
Do not enter overly detailed medical records, sensitive images or unnecessary health data in the note field.
If the appointment needs to be changed, contact the hotline as early as possible so the clinic can assist.
When visiting, bring identification documents and relevant medical records if available.
The actual appointment time may vary depending on patient volume, clinical needs and the clinic’s reception process.
